Waiver / liability release

1. Acknowledgment of Risk

I, the undersigned, understand that participation in exercise, fitness training, and use of gym equipment at this 24-hour facility involves inherent risks. These risks include, but are not limited to: muscle strains, sprains, broken bones, cardiovascular events, slips, falls, equipment malfunction, or other injuries which may result in serious injury or death. I voluntarily choose to use this facility with full knowledge of these risks.

2. Assumption of Responsibility

I accept full responsibility for my own health and safety while using the gym, regardless of the time of day. I understand that staff may not be present, and emergency response may be delayed during unstaffed hours. I agree to use the facility responsibly, to follow all posted rules, and to immediately report any unsafe conditions or malfunctioning equipment.

3. Release and Waiver

In consideration for being allowed access to this facility, I hereby release, discharge, and hold harmless IronHide Fitness LLC, its owners, officers, employees, contractors, and agents from any and all claims, demands, causes of action, damages, or liabilities, whether known or unknown, arising out of my use of the facility and participation in activities, even if caused in whole or in part by the negligence of the released parties.

4. Medical Authorization

I certify that I am physically able to participate in exercise activities and have consulted with a physician if necessary. In the event of a medical emergency, I authorize IronHide Fitness LLC to obtain emergency medical treatment for me, and I agree to be financially responsible for any related expenses.

5. 24-Hour Access Rules

I will not allow any other person to use my access card/key/fob.
I will not grant entry to anyone who does not have authorized access.
I understand that surveillance cameras may monitor the facility at all times.
I will not damage equipment, mirrors, walls, flooring or fixtures.
I will not deface walls or equipment.
Violation of these rules may result in immediate termination of access without refund.
Children 13 and under must be accompanied by an adult.

6. Governing Law

This agreement shall be governed by the laws of the state of West Virginia. If any portion of this waiver is found to be invalid, the remaining provisions shall continue in full effect.
